| 目的 探讨临床药师进行出院用药指导对提高低文化水平出院患者用药安全性、依从性的影响。方法 临床药师在临床给患者用药指导中，对内科住院接受治疗的年龄>60岁而文化水平低于小学的出院患者进行了特殊的用药指导，设计了用药记忆信息卡提醒患者用药。结果与结论 药师根据患者的用药依从性、病情维持情况及用药知识情况对低文化水平患者出院用药进行指导，有助于提高用药依从性、有效性、安全性，提高患者病情控制效果。 |

| OBJECTIVE To explore the influence of medication guidance from clinical pharmacist on safety and compliance of dischanged patients with low cultural level. METHODS The discharged patients above 60 years old without primary school education received a special medication guidance. Clinical pharmacists designed medication cards for reminding them of administration. RESULTS & Conclusion Base on the elderly discharged patients’ compliance, maintenance and drug knowledge, the medication cards designed by clinical pharmacists can help to improve effectiveness and safety of disease control. |
