摘要: |
目的 探索氨磺必利血药浓度存在高度个体差异来源,为临床个体化治疗提供参考。方法 从PubMed数据库、Web of Science核心期刊数据库、中国知网(CNKI)和万方数据库以“amisulpride”和“concentration”或“氨磺必利”和“血药浓度”等关键词进行检索,并分析氨磺必利血药浓度的影响因素。结果 最终纳入49项氨磺必利血药浓度研究,33项研究的综合结果提示日剂量与血药浓度呈正相关。血药浓度或剂量校正浓度的影响:41%(9/22项)研究提示不同合并用药(3项锂剂、2项氯氮平、1项丙戊酸、3项其他)的潜在影响,25%(3/12项)研究提示女性剂量校正浓度高于男性,69 %(9/13项)研究提示血药浓度随年龄增加而升高, 2项关于体重指数研究具有相反结果,2项研究提示血清肌酐与血药浓度的正相关,9项研究提示药物转运体及其基因多态性存在潜在影响。结论 为保证氨磺必利在临床应用的安全有效,可根据性别、年龄和肌酐清除率进行个体化剂量调整。另外,要根据中国人群遗传特点,开展临床研究进一步验证药物转运体及其基因多态性对氨磺必利血药浓度的影响。 |

OBJECTIVE Aims to determine the sources of highly individual variation in amisulpride plasma concentrations to guide individualized clinical treatment. METHODS The keywords "amisulpride" and "concentration" were searched from PubMed database, Web of Science, China National Knowledge Infrastructure and Wanfang database. A review was performed to analyze the factors influencing plasma concentration of amisulpride. RESULTS 49 studies on the plasma concentration of amisulpride were included. The combined results of 33 studies suggest a positive correlation between daily dose and drug plasma concentrations. The effects of drug plasma concentration or dose-corrected concentration were presented: 41% (9/22) studies suggested potential effects of different combinations of medications (3 lithium, 2 clozapine, 1 valproic acid, 3 other), 25% (3/12) studies suggested that women were higher than men, 69% (9/13) studies showed an increase with age, 2 studies on body mass index had opposite results and 2 studies suggest positive correlation between serum creatinine and drug plasma concentration. Finally, 9 studies suggested the potential impact of drug transporters and their genetic polymorphisms. CONCLUSION In order to ensure the safety and efficacy of amisulpride in clinical applications, individualized dosage adjustment can be performed for special populations according to gender, age and creatinine clearance. Furthermore, future research should focus on drug transporters and gene polymorphisms specific to Chinese populations to validate the effect on the plasma concentration of amisulpride. |
