非抗感染目的长程应用阿奇霉素的安全性研究现状

摘要:
阿奇霉素以往通常用于短程抗感染治疗,然而近年来发现它具有多方面的抗菌外效应,国外多个指南推荐用于哮喘、支气管扩张、慢性阻塞性肺疾病等呼吸道疾病的辅助治疗中。由于这种以非抗感染目的长程应用阿奇霉素的给药方式,具有用药疗程长、适用范围广等特点,其存在的潜在安全性问题(如细菌耐药性增加、心脏毒性、听力受损等)备受关注,也颇具争议。本文旨在阐述阿奇霉素在长期应用过程中涉及安全性方面的研究现状,以期客观和正确认识其潜在风险,为临床决策提供参考。

Abstract:
Azithromycin was usually used in short-term anti-infection therapy in the past. However, azithromycin has been found to have many non-antibacterial effects in recent years. Many foreign guidelines recommend it for the adjuvant treatment of respiratory diseases such as asthma, bronchiectasis and chronic obstructive pulmonary disease. Due to the long-term application of azithromycin for the purpose of non-antibacterial effects, it has the characteristics of long treatment course and wide application range. Therefore, its potential safety problems(such as increased bacterial drug resistance, cardiotoxicity, hearing loss, etc.) have attracted extensive attention and controversial. This paper aims to describe the research status of safety of azithromycin in the process of long-term application, in order to objectively and correctly understand its potential risks and provide help and reference for clinical decision-making.
